What is palliative care? Palliative care is a form of care that focuses on pain and symptom management. Our goal is to help better a patient and family from their life-limiting illnesses.	What is hospice care? Eternal Hope provides hospice care to anyone who has been faced with a life-limiting disease. Hospice care is given to patients with a terminal diagnosis and a prognosis of six months or less.
When is it appropriate for palliative care? A person who is suffering from a life limiting illness may come on our palliative care services at any time.	When is it time for hospice care? Hospice is recommended to anyone who’s life-limiting illness can no longer be treated. Often times hospice care is not administered soon enough, because people may believe hospice is just “giving up”. However, that is not the case when it comes to hospice care provided by Eternal Hope. We believe in offering hope and bringing quality care to our patients and families.
How can you benefit from our palliative care program? Palliative Care aims to improve the quality of life. We provide this service within the home.	How can you benefit from our hospice care? We are available at your home or in any facility that you reside. An informational visit from one of our experienced staff members can be requested by you, a family member, or a caregiver. At this visit all of your questions and concerns will be addressed. With your physician’s consent, services can be initiated.
How is palliative care paid for? Palliative care is covered by Medicare Part B and some secondary insurance.	How is hospice paid for? Costs for hospice services are typically covered by Medicare, Medicaid, and most primary insurances.

Our Hospice Care Services

Home Care

Our philosophy is designed to allow a patient to reside within their own home, an assisted living, or nursing facility. The hospice team will provide supportive care tailored to meet the unique needs of the patient and family.

General Inpatient Care

When symptoms can not be managed in the home, a patient may utilize our inpatient hospice unit at The Hope House, which offers around the clock skilled nursing care. Our hospice physician and nurse case manager will determine the need for these services.

Respite Care

When a caregiver requires a brief break from the caregiving role, a respite stay may be available in our inpatient facility depending on availability

Continuous Care

If uncontrolled pain or symptoms emerge for a patient, crisis management may be provided within the home by skilled staff as determined by the interdisciplinary team.
